Before attending any team activity, Calvin requires you to complete a COVID Safety screening. If an athlete is not feeling well, we ask that you stay home and not participate or use any Calvin facilities. You will be required to verify the following items before each practice/competition:

 

1. Current date

2. Athlete’s full name (First and Last)

3. Symptoms (if any)

4. Compliance with MDHHS protocols

 

Remember that in addition to filling out the COVID Safety Form, all athlete’s will be temperature checked before each practice. Please use the link below to quickly complete the required safety screening.
